How patients are enrolled in Echo Health SMS care plan check-ins

Echo Health uses a provider-mediated consent model. Patients are enrolled by their healthcare provider during a clinical visit. The patient then accepts consent through the Echo Health mobile application before any care coordination messages are sent. This page documents the complete opt-in journey.

Provider Enrolls Patient

During discharge planning or a care coordination visit, the healthcare provider opens the Echo Health coordinator portal and selects “Add Patient.” The provider enters the patient's name, email address, and mobile phone number.

Provider Selects SMS Delivery

The provider selects how the consent invitation should be delivered to the patient. Options include Email, SMS, or Both. This determines the delivery method for the invitation — not the consent itself.

Provider Selects Consent Scopes

The provider selects which data access and feature scopes the patient will be invited to consent to. These scopes control access to clinical data and in-app features. SMS care plan check-ins are part of the care coordination workflow and are not controlled by these scopes.

Patient Receives Consent Invitation

The patient receives an SMS (or email) with a one-time verification code and a link to complete enrollment. This message is the first communication the patient receives from Echo Health.

Patient Reviews and Accepts Consent

The patient opens the Echo Health mobile app (or taps the invite link), enters their verification code, and reviews the consent scopes their provider has requested. The patient then accepts or declines each scope. Consent decisions are made by the patient, not the provider.

The provider portal cannot accept consent on behalf of the patient. This is designed so that patient consent is genuine and patient-controlled.

Message frequency varies based on your care plan. Message and data rates may apply. Reply STOP to opt out at any time. Reply HELP for assistance.

You can change your consent at any time in Settings

How SMS Consent Is Established
  1. The provider records the patient's phone number during enrollment.
  2. The patient receives a verification message via SMS.
  3. The patient completes OTP verification, confirming they control the phone number.
  4. The patient reviews consent scopes and the SMS check-in consent checkbox (unchecked by default).
  5. The patient actively checks the SMS consent box and taps Accept.
  6. Consent is recorded with a timestamp. SMS check-ins begin only after this explicit consent.
  7. Every message contains STOP instructions for immediate opt-out.
  8. The patient can revoke SMS consent at any time in Settings.

Care Coordination Messages Begin

Only after the patient has accepted consent are care plan check-in messages sent. These messages ask about specific care plan tasks assigned by the provider. The patient can reply Y or N. The patient can reply STOP at any time to opt out of all messages.

At no point does the provider accept consent on behalf of the patient. The patient controls their own consent decisions through the mobile application and can revoke consent or reply STOP at any time.

Message Frequency

Message frequency varies based on the patient's care plan and their provider's instructions. The active care plan period is typically 30 days.

Opt-Out

Patients can reply STOP to any message at any time. One confirmation message is sent and no further messages follow. Opting out does not affect the patient's care plan or relationship with their provider.
